Contact and release form signed by a parent/guardian Please review and sign the following document by clicking here.
Signed copy of Hope Reins Camp Rules and Dress Code A parent or guardian must review these documents with each camper and must sign as proof of review. Click here to access the document.
Physician’s Release signed by camper’s physician Please have your camper’s physician sign this release stating that equine activities are approved and that there are no contraindications for the camper to participate in equine activities. Click here to access the document. If you are currently a client of Hope Reins in Texas and your forms are up-to-date, this is not applicable.
Completed Activities of Daily Living Form A parent or guardian must complete this document detailing each camper’s activities/needs for daily living and interaction. Click here to access the document.
Confirmation of payment- Payments can be completed by clicking the button below or can be made by check to Hope Reins in Texas. Please mail checks to the Hope Reins mailing address at: P.O. Box 57 Bulverde, Texas 78163.

-Some restrictions may apply to horse riding time and other activities due to a camper’s limitations.
-We desire your camper’s success, as well as the cohesiveness of all campers and their success; therefore, acceptance to the camp is subject to Hope Reins in Texas professional evaluation and approval.
-All camp applications are subject to available camp spots.
